{"id":451,"date":"2018-04-30T18:04:57","date_gmt":"2018-04-30T22:04:57","guid":{"rendered":"https:\/\/digital.hbs.edu\/platform-mhcdsolutions\/submission\/new-innovation-primary-care-models-of-care\/"},"modified":"2018-04-30T18:05:54","modified_gmt":"2018-04-30T22:05:54","slug":"new-innovation-primary-care-models-of-care","status":"publish","type":"hck-submission","link":"https:\/\/d3.harvard.edu\/platform-mhcdsolutions\/submission\/new-innovation-primary-care-models-of-care\/","title":{"rendered":"New Innovation \u2013 Primary Care Models of Care"},"content":{"rendered":"<p><span style=\"color: #333333;font-family: 'Georgia','serif'\">Health care is changing and there is a demand to reduce health care spending in the US.\u00a0 One way health care systems are attempting to reduce healthcare spending is by reducing unnecessary ED and hospital utilizations.\u00a0 The goal is to provide\u00a0more preventive care and chronic disease support as well as increasing access to urgent and same day appointments.\u00a0 One of our challenges is that we have a shortage of primary care physicians and the current models of care are designed for &#8220;sick patients&#8221;&#8216; and not\u00a0&#8220;well patients&#8221;.\u00a0 \u00a0One of our health care system goals\u00a0are to\u00a0re-imagine\u00a0Primary Care\u00a0using new innovative models to meet the needs for healthcare for our\u00a0community.\u00a0 This means introducing new models of care\u00a0that will encourage population health\u00a0by incorporating community outreach and preventative care.\u00a0 The goals of\u00a0 implementing new models of care is to improve access, reduce\u00a0total cost of care,\u00a0increase panel sizes while maintaining quality, satisfaction, and staff and physician engagement and wellbeing.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"color: #333333;font-family: 'Georgia','serif'\">One of way of meeting this challenge is to incorporate more innovative models and approaches to\u00a0primary care such as a\u00a0team-based approach that will include adding additional staff such as MA&#8217;s, RN&#8217;s,\u00a0NP&#8217;s, educators, behavior health specialists and pharmacists\u00a0to the physician office practices and at the same time decrease the need for adding additional physicians.\u00a0\u00a0\u00a0 Another model is virtual health care where care is performed virtually.\u00a0 We are planning to open a practice of the &#8220;future&#8221; to incorporate one of these models of care. \u00a0 It would be great to have feedback from those who may have implemented one of these models of care and hear the successes and the failures.\u00a0 <\/span><\/p>\n<p><span style=\"color: #333333;font-family: 'Georgia','serif'\">The question is how do we implement these new models of care\u00a0in preparation for tomorrow&#8217;s population health model in today&#8217;s\u00a0fee for service model?
